Inclusion Criteria

Live within 50 mile (max 80 mile) radius of Escondido, CA
Diagnosed with any type of Diabetes and/or Cardiovascular Risks
Diagnosed with cardiovascular risks such as Coronary artery disease (CAD), Cerebrovascular disease, Heart Attack, Stroke, or Peripheral arterial disease (PAD)
Each study has specific requirements such as age, sex, and current diagnosis of the medical condition, medical treatment, lab results, etc
Study staff will determine eligibility based on additional study specific criteria
