Summary

The purpose of CLOVER is to utilize Epic Healthy Planet to increase adherence to United States Preventive Services Task Force (USPSTF) and Centers for Disease Control and Prevention (CDC) recommendations in adults age 50 and older.

Inclusion Criteria

Is your age between 50 yrs and 80 yrs?
Gender: Male or Female
Do you have any of these conditions: carcinoma lung or Pulmonary Disease or adiposity or Chickenpox (Varicella Zoster Infection) or Malignant neoplasm of colon or zona or colon carcinoma ...?
Do you have any of these conditions: Herpes Zoster or zoster or carcinoma lung or lung carcinoma or Lung Neoplasm or Lung Disease or Colon Cancer Screening or colon carcinoma or Hepatitis...?
Do you have any of these conditions: Tobacco Use Cessation or Colon Cancer Screening or Colorectal Cancer or Pulmonary Disease or Shingles or Herpes Zoster Infections or Bronchial Neoplas...?
Any patient who has a UC Davis or UC Irvine primary care provider (PCP) and is deficient in any of the following: colon cancer screening, lung cancer screening, tobacco cessation counseling, obesity counseling, pneumonia vaccination, shingles vaccine, and hepatitis C screening

Exclusion Criteria

Patients outside of the age limits or have an established PCP
