Carolinas Healthcare System selects Cerner’s Population Health Management Platform

Cerner, a global provider of healthcare technology, announced Carolinas HealthCare System (CHS) has chosen Cerner’s HealtheIntentSM population health management platform to coordinate and manage care for its more than 12 million patient interactions each year across its facilities and clinically integrated network, Carolinas Physician Alliance (CPA). CHS will continue to use the Cerner Millennium integrated electronic health record (EHR) and will also extend its remote hosting relationship with Cerner.

HealtheIntent is designed to aggregate data in near real-time from multiple sources to create a single, comprehensive view of an individual’s experiences across the continuum of health and care.

“Carolinas HealthCare System is committed to delivering the most convenient, quality care possible to the patients and communities we serve,” said Ruth Krystopolski, senior vice president, population health, Carolinas HealthCare System. “Our goal is to equip our care teams with a holistic and timely picture of a patient’s interactions throughout CHS, as well as provide system wide insights to ultimately improve quality of care and reduce costs for our entire patient population.”

“As the healthcare industry transitions into value-based care, the ability to consume all data whether it originates from our own or our affiliate’s electronic medical records, social determinants of the patient, self-reported data or from any source becomes invaluable,” said Craig Richardville, chief information and analytics officer at Carolinas HealthCare System. “This allows us to create a 360 view of each patient to offer the insight to our care team to deliver appropriate treatment, prevention and to bend the cost curve to make sure our services are more affordable to the patient and increase the health of each patient and of our communities.”   

This comprehensive suite includes: HealtheEDWSM, Cerner’s enterprise data warehouse solution; HealtheRegistriesSM, a registries and scorecards solution designed to identify gaps in care and recommend interventions; and HealtheCareSM, Cerner’s community care management solution that identifies, stratifies and prioritizes individuals for care management services.
