This study will evaluate assessment of NT-proBNP levels before discharge on management and clinical outcomes in patients admitted with heart failure, and evaluate the use of NT-proBNP measurement in managing the patients followed in a heart failure clinic.

- Adult patients ≥18 years of age;
- Clinical stable heart failure, NYHA I-III.
Exclusion Criteria:
- Candidates for surgical interventions;
- Severe renal dysfunction.
