Who we are:

Atrius Health is the Northeast’s largest nonprofit independent multi-specialty medical group. The Atrius Health practices including Dedham Medical Associates, Granite Medical Group, Harvard Vanguard Medical Associates—together with VNA Care Network & Hospice— serve 675,000 patients across 30 clinical locations in eastern Massachusetts. A national leader in delivering high-quality, patient-centered coordinated care, the Atrius Health medical groups and home health agency & hospice work together, and in collaboration with hospital partners, community specialists and skilled nursing facilities, to develop innovative, effective and efficient ways of delivering care in the most appropriate setting, making it easier for patients to be healthy.

Four leading community-based groups have joined together as Atrius Health, a non-profit alliance, to transform the delivery of health care in eastern Massachusetts. Building on our long and proud history of high quality care, Dedham Medical Associates, Granite Medical Group, Harvard Vanguard Medical Associates, and VNA Care Network Foundation , are working together to develop better ways to coordinate care across multiple settings and find new and better ways to better coordinate home health services with ambulatory care. All groups utilize an electronic medical record to improve care coordination and communication for patients.

Atrius Health medical groups serve 675,000 adult and pediatric patients in over 2.2 million visits annually to 42 practice locations. Our medical groups include more than 750 physicians and a total of almost 6,800 employees.

We offer over 35 specialties, from including obstetrics, to pediatrics, including oncology, cardiology, ophthalmology, sports medicine, allergy, dermatology, surgery and behavioral health - as well as labs, imaging and pharmacies which are conveniently located on-site in many of our offices.

Leading the Way in Accountable Care: Managing Quality and Cost Across the Continuum of Care

Atrius Health has a unique history in managing quality and cost for our patients across all settings where they are receiving care.

Accountable Care
Atrius Health has accountable care contracts in place with Blue Cross Blue Shield (Alternative Quality Contract – AQC), Harvard Pilgrim Health Care and Tufts Health Plan for commercial insurance patients; Neighborhood Health Plan for Medicaid patients; and Tufts Health Plan Medicare Advantage for Medicare Advantage patients. Under these contracts, Atrius Health coordinates care for each plan’s members using leading-edge population management methods and patient outreach for preventive services and chronic disease management. Atrius Health has demonstrated significant improvement in measured clinical quality and reducing total medical expense.

Medicare Pioneer Accountable Care Organization (ACO)
In 2012, Atrius Health was chosen as one of 32 leading healthcare organizations to participate in the Pioneer Accountable Care Organization (ACO) model, a transformative initiative sponsored by the Centers for Medicare & Medicaid Services (CMS) Innovation Center. Through the Pioneer ACO model, Atrius Health works with CMS to provide Medicare beneficiaries with higher quality care, while reducing growth in Medicare expenditures through enhanced care coordination. In 2014, Atrius Health had the highest scores on quality measures among Pioneer ACOs in New England and the second highest quality scores compared to all Pioneer ACOs in the country based on CMS’s 33 ACO quality measures.

Patient-Centered Medical Home
Many in the industry view the Patient-Centered Medical Home, in which a multi-disciplinary primary care team coordinates care broadly for its patients, as the primary care office of the future. Atrius Health primary practices have received the highest possible national certification as Level 3 Patient-Centered Medical Homes from The National Committee for Quality Assurance (NCQA), making Atrius Health one of the largest group practices in the country to achieve NCQA medical home designation. As part of caring for our patients, Atrius Health provides 24/7 telephone medical advice and treatment through our advance practice clinicians and nurses in addition to extensive evening, weekend and holiday urgent care hours.

Health Information Technology

Electronic Health Records
Atrius Health clinicians and their support teams utilize an advanced electronic medical record (Epic®) to integrate and coordinate care electronically across specialists and different healthcare sites. As an early and leading edge user of the Epic system, clinicians across Atrius Health benefit from strong decision support tools linked to efficient workflows. Atrius Health medical teams utilize an online patient portal through Epic which allows patients to access portions of their medical record online, directly schedule appointments, receive lab test results, and communicate with their care team via a secure connection through a desktop or mobile device. In 2013, Atrius Health was the first ambulatory-only practice to earn Stage 7 recognition from HIMSS (Healthcare Information and Management Systems Society) Analytics. Stage 7 represents the highest level of EMR adoption and indicates a health system’s advanced use of an electronic patient record environment.

Clinical Informatics
Atrius Health has built a sophisticated data warehouse that combines the electronic medical records, claims data, and pharmacy data for hundreds of thousands of patients. This data warehouse has become a tremendous data source for leading edge research, practice improvement, and enhanced patient care.

HIT Interoperability
One of the foundational elements of health care reform is the safe and secure exchange of electronic health information between organizations. Working with Epic and its hospital partners, Atrius Health has developed a web portal that allows clinicians within both organizations to have easy one-click, read-only access to a patient’s electronic medical record at the other facility. This allows Atrius Health clinicians to better monitor and manage patients by preventing unnecessary duplication of testing and ensuring that patients receive the follow-up care they need to prevent hospital readmission. Atrius Health is also a participant in the Mass HIway, a statewide electronic health information exchange that allows healthcare providers to share patient records.

Our research population includes rural, urban and suburban patients ranging from infants to the elderly. Atrius Health clinicians recognize the success of every clinical trial hinges on the success of the recruiting strategies for identifying qualified and compliant patients. We have organized thousands of our patients into a computerized database allowing rapid retrieval of patients based on their diagnosis.
