Conveniently Located

Forward Clinical Trials has one location in the Tampa Bay Area. The Tampa office is a 6,500 square foot research facility located within a Dermatology practice.

Located 10 minutes from St. Joseph's Hospital on Van Dike Road and only 20 minutes from Tampa International Airport.

Forward Clinical Trials is led by Seth B. Forman, MD. Dr. Forman has been voted the "Best Dermatologist" in North Tampa three years in a row, 2011-2013. Forman Dermatology is general, surgical and cosmetic practice that generates approximately twenty thousand (20,000) patient visits a year through our facility.

The practice provides an array of services including:

  • nUVB (Narrowband Ultraviolet B) phototherapy
  • Targeted PUVA (Psoralen + Ultraviolet-A) phototherapy
  • Targeted nUVB
  • Low dose radiation therapy for non-melanoma skin cancers and keloids
  • Blu-light for actinic keratosis and acne
  • Cosmetic consultation and non-surgical treatments

Additionally, Dr.Forman services over one hundred and fifty (150) skilled nursing facilities (SNF's), assisted living facilities (ALF's) and independent living (IL's) residences in Florida. This travel practice visits residents in Hillsborough, Pinellas, Pasco, Hernando, Sarasota, Manatee, Polk and Marion Counties.

The home facility also houses a full service pathology laboratory. Our lab is able to process routine pathology, special stains and immunostains.

Facility Description

Forward Clinical Trials is a access-controlled 6,500 square foot research facility located within a Dermatology practice that offers outpatient services and is capable of conducting Phase II – IV studies. The Dermatology practice provides services for over 150 Nursing Home facilities. Specialized equipment on hand at our facility includes a -20 and -70 freezer, refrigerators, 2 centrifuges, an EKG machine, all of which are calibrated on an annual basis. We have a full Pathology Laboratory, CLIA certified and a Research Lab with a current CLIA Waiver. Our site has comprehensive proprietary SOPs, and research staff have experience with multiple Electronic CRF’s (eCRF) systems.

We have limited-access, temperature-controlled investigational medication storage and lab supplies, accessible only to research personnel. Access to public transportation. Services also include Television, CD/DVD players for patient education/entertainment, and snacks.

Seth B Forman, MD - Dermatologist

The principal investigator and President/CEO, Dr. Seth Forman, graduated medical school at the Medical College of Virginia in Richmond, VA. He then completed his residency in dermatology, a training program dedicated to dermatology and skin care, at the Medical College of Virginia.

Dr. Forman has been featured for his innovative techniques and expertise on the "The Doctors", MSNBC, and the local ABC, CBS and NBC affiliates in the Bay Area. Dr. Forman has also provided expertise in Glamour Magazine, The Tampa Bay Times, and The Tampa Tribune. His ability an interest in the national and local news media is a bonus for recruiting. He is known as the go to guy in the Tampa Bay area and beyond.

Dr. Forman also serves as medical director for Psoria-Shield, a wholly owned subsidiary of Wellness Centers USA. He has successfully treated patients with complicates cases of Psoriasis, Eczema, Vitiligo and Alopecia Areata.

Forward Clinical Trials, led by Dr. Forman, has enrolled patients in trials for common conditions such as psoriasis and psoriatic arthritis, however Forward Clinical Trials, strives to enroll the difficult trials, including Hidradenitis Suppurativa.

Throughout his medical training, Dr. Forman was committed to academic pursuits. He has researched, studied and written peer-reviewed articles about various dermatological issues including melanoma, basal cell carcinoma, merkel cell carcinoma, vascular tumors, and inflammatory conditions such as eczema.

Dr. Forman also is the primary author of two chapters in Comprehensive Dermatologic Drug Therapy, the leading reference textbook for dermatological therapeutics.

Jonathan Forman, MD

Dr. Jonathan Forman graduated from the University of South Florida College of Medicine. He completed his residency at the University of Medicine and Dentistry of New Jersey in a training program dedicated to Otolaryngology, sinus allergy and pulmonary diseases.

Dr. Jonathan Forman was selected and served as Chief Resident during his final year of training.

Throughout his medical training, Dr. Jonathan Forman was committed to academic and research pursuits with interests in sinusitis, nasal polyps, allergic diseases and pulmonary diseases.

Stephen J. Szabo, MD, PA
Psychiatrist
Diplomate of the American Board of Psychiatry and Neurology.

Dr. Stephen Szabo graduated from the Ohio State University College of Medicine. He began his residency at the Ohio State University Department of Psychiatry. He served as Chief Resident at the University of South Florida Department of Psychiatry. Dr. Szabo’s training program focused on depression, anxiety, attention deficit hyperactivity disorder (ADHD), Alzheimer’s, Parkinson’s, Bipolar and Schizophrenia.

Dr. Szabo has served as an investigator on multiple trials at Forward Clinical Trials.

Dr. Szabo served on the faculty at the University of South Florida College of Medicine in the Department of Psychiatry.
