Summary: Childhood Cancer Survivor Study
LIVING SUBJECTS: You have/your child has been asked to participate in this research study because you are/your child is a five or more year survivor of childhood cancer. The purpose of this study is to research the health status of individuals who were treated for childhood cancer in order to learn how to treat and follow children diagnosed with cancer in the future. Your/your child’s participation in this study is expected to last the remainder of your/your child’s life or as long as you are/your child is willing to participate in this study.
NEXT OF KIN:You have been asked to participate in this research study because your deceased child/spouse/close relative was a five or more year survivor of childhood cancer. The purpose of this study is to research the health status of individuals who were treated for childhood cancer in order to learn how to treat and follow children diagnosed with cancer in the future. Your participation in this study is expected to last about 45 minutes on one occasion.
