Summary: A City of Hope Protocol for Collecting, Banking and Release of Human Biological Materials for Research

You have/your child has been asked to participate in this research study because you/your child will undergo procedures, such as a blood draw or surgery, to collect blood or tissue specimens as a City of Hope patient.  The purpose of this research study is to acquire and bank biological specimens and related information from medical records for research to improve understanding of cancer biology and response to treatment.  Your/your child’s participation in this study is expected to last for 30 minutes on one occasion.  Medical information will be collected by looking at your/your child’s medical record and by mailing you/your child a letter or calling you/your child on the telephone periodically to see how you are/your child is doing. If they need to call you/your child on the telephone, the call would last about 5 minutes.
