Summary: Prostate Cancer Database

You have been asked to participate in this research study because you have prostate cancer and are being treated at the City of Hope National Medical Center (COH).  The purpose of this study is to develop a database (a collection of data on treatments and outcomes from a large group of patients), which will be used to evaluate the patterns of prostate cancer care and results in men being treated at COH.  Your participation in this study is expected to last indefinitely, as information will be obtained from your medical records throughout your lifetime.
