Summary: Are you a male at least 30 years old?

Do you have at least one of the following diseases?

  • High blood pressure
  • High Cholesterol
  • Metabolic syndrome
  • Coronary artery disease
  • Diabetes
  • Depression
  • Lower Urinary Tract Symptoms (such as urgent or frequent urination, a poor urine stream or night-time accidents)

OR

  • Do you smoke?
  • Are you obese?
  • Is your waist more than 40 inches?

OR: Do you take any of the following medications?

  • Insulin
  • Lipitor
  • Elavil
  • Accupril
  • Lisinopril
  • Flomax
  • Glipizide
  • Zocor
  • Effexor
  • Diltiazem
  • Norvasc
  • Uroxatral
  • Metformin
  • Paxil
  • Zoloft
  • Atenolol
  • Lotensin
  • Cardura
  • Glucophage
  • Prozac
  • Wellbutrin
