Summary: Do you have Crohn’s disease or ulcerative colitis?

If you answer "YES," you may be able to help researchers at the University of Kentucky who are trying to locate close relatives of patients like you.

You are eligible to participate in our research if you:

• have Crohn’s disease or ulcerative colitis;
• are age 14 or older; and
• would like for us to contact by mail the relatives you name on your survey to see if they would like answer health questions for our research study.

This is a mail-back survey. Postage return is pre-paid.
