Long Term Follow-Up After Breast Cancer

You have been asked to participate in this research study because you were diagnosed with breast cancer and at least a portion of your therapy is being received/was received at City of Hope. The purpose of this study is to define comprehensive long-term follow-up care for breast cancer survivors at the City of Hope. Another purpose of this study is to help researchers better understand health-related quality of life and long-term complications in breast cancer survivors. Your participation in this study is expected to continue in an ongoing manner.
