Passport for Care: Implementation in Multiple Institutions
You have/your child has been asked to participate in this research study because you are/your child is a survivor of childhood cancer and have/has been off treatment for at least 2 years. The purpose of this study is to evaluate an Internet-based information system (known as the “Passport for Care”) that is designed to provide healthcare providers with computer-generated guidelines for follow-up care of childhood cancer survivors that are individualized based on cancer treatment history. Your/your child’s participation in this study is expected to last indefinitely.
